Dr. Leon Lewis, Dr. Paul Holmwood
and Theodora Lewandowski form Eye Care & Vision Associates,
Inc., a medical service organization (MSO) designed to
provide space, staffing, equipment, supplies and administrative
services for the doctors' ophthalmology practices. The
office is located at One Hopkins Road in Williamsville. |

A group practice is established when
Drs. Lewis and Holmwood form a partnership called Eye Care & Vision Associates,
LLP (ECVA). Eye Care & Vision Associates, Inc.
provides a "turn-key" operation for Eye Care & Vision
Associates, LLP. |

An optical dispensary opens at the
Hopkins Road office in Williamsville. |
| December 1998 |
Eye Care & Vision Associates, LLP opens an office in
Hamburg, taking over the space formerly occupied by Dr. Malcolm
McIsaac. Dr. Holmwood provides services at this location
along with Dr. McIsaac until Dr. McIsaac's retirement in
August of 1999. |
| July 1999 |
Dr. George W. Pfohl joins the group,
providing ECVA with its third location: on Elmwood Avenue
in downtown Buffalo. |
| September 1999 |
Dr. Thaddeus A. Zak, a pediatric ophthalmologist
with a practice located in the building adjacent to ECVA's
Williamsville
office, retires. Eye Care & Vision Associates,
LLP absorbs a large portion of Dr. Zak's patients, greatly
expanding our base of younger patients. |

Eye Care & Vision Associates,
LLP acquires the optical dispensary located in Dr. Pfohl's
former office on Elmwood
Avenue. |
| April 2000 |
ECVA undergoes a major expansion and
relocation in the southtowns by acquiring a lease on a
4,500 square foot building on Southwestern
Blvd. near McKinley Parkway. ECVA totally renovates the office,
outfitting it with six examination rooms and a minor surgery
room. |
| May 2000 |
ECVA hires Dr. William Butka, an optometrist.
Dr. Butka divides his time between each of our two locations. |
| June 2000 |
Eleanor Lewis, a certified orthoptist,
is hired. |
| August 2001 |
ECVA adds its third optical shop, at the Hamburg office. |
November 2001 |
ECVA expands into Niagara County by
purchasing the practice of Dr. Robert J. Gormley on Williams
Road in Niagara Falls,
NY. |
| January 2002 |
Dr. William O. Sass, who shared office
space with Dr. Gormley, also begins transitioning his practice
to the doctors of
ECVA. Both Drs. Gormley and Sass still see patients
at the Niagara Falls office; Dr. Gormley as an ECVA employee
and Dr. Sass as a sole practitioner using the services of
the MSO. |
July 2002 |
Dr. David P. Montesanti, an ophthalmologist,
is hired, and divides his time between ECVA's Elmwood Avenue
and Niagara
Falls offices. |
| December 2002 |
John T. Hammersmith, O.D. (who had
been working for ECVA on a part-time basis since 1998)
begins working full time
for ECVA, dividing his optometric practice between ECVA's
Williamsville, Elmwood and Niagara Falls offices. |
| April 2003 |
Leslie A. Long, O. D. is hired, and
sees patients at ECVA's Downtown Buffalo, Hamburg and Williamsville
offices. |
| May 2003 |
ECVA now boasts four state-of-the-art
offices and three optical shops throughout Western New
York. There is
a doctor available to see patients
at all four locations Monday through Friday. |
| April 2005 |
ECVA relocates its Niagara Falls
office from Williams Road to a newly-built, 3,700-square-foot
facility just one mile away. |
| September 2005 |
ECVA adds its fourth optical shop, at the Niagara Falls office. Joseph Miller, ophthalmic dispenser and contact lens practitioner, is hired to manage the shop. |
